Minor candidate

Check what documents are needed if for registration comes:

  • identity document of the candidate;
  • identity document of the parent;
  • original declaration of the legal representative - a template available in the section Downloads;
  • candidate's birth certificate* together with:
    • sworn translation into Polish;
    • apostille or legalization - the birth certificate issued in EU member country is exempted from the obligation to attach legalization/apostille.

* Instead of the birth certificate, the parent can show his/her mObywatel application which confirms that he/she is the candidate's parent.

The recruitment documents necessary for the registration are signed by the underage candidate and the parent.

  • identity document of the candidate;
  • identity document of the legal/temporary guardian;
  • original declaration of the legal representative - a template available in the section Downloads;
  • court decision appointing the legal/temporary guardian.

  • candidate's birth certificate* together with:
    • sworn translation into Polish;

  • original declaration of the legal representative with a notarized signature of the parent (the parent signs the declaration in the presence of a notary) - a template available in the section Downloads. If the document is made outside of Poland, additionally the following should be provided:
    • sworn translation into Polish of the above-mentioned document,
    • apostille or legalization for notarial certification;
  • candidate's birth certificate together with:
    • sworn translation into Polish;
    • apostille or legalization - the birth certificate issued in EU member country is exempted from the obligation to attach legalization/apostille. 

The recruitment documents necessary for the registration are signed by the underage candidate.

    • attorney’s identity document;
    • original declaration of the legal representative with a notarized signature of the parent (the parent signs the declaration in the presence of a notary) - a template available in the section Downloads.If the document is made outside of Poland, additionally the following should be provided:
      • sworn translation into Polish of the above-mentioned document,
      • apostille or legalization for notarial certification;
    • candidate's birth certificate together with:
      • sworn translation into Polish;
      • apostille or legalization - the birth certificate issued in EU member country is exempted from the obligation to attach legalization/apostille;
    • power of attorney granted to the attorney by the candidate’s parent in the form of a notarial act or power of attorney with a notary confirmation of the authenticity of the signature of the parent. The power of attorney should indicate activities that may be performed by the attorney in connection with the recruitment process. If the document is made outside of Poland, additionally the following should be provided:
      • sworn translation into Polish of the above-mentioned document,
      • apostille or legalization for the power of attorney document;
    • signed by the attorney: consent to the processing of personal data and information of the personal data administrator.

    The recruitment documents necessary for the registration are signed by the attorney.
